Fiona Dourif is making a significant leap from the emergency room to the supernatural, joining the growing cast of “A Head Full of Ghosts,” an upcoming horror film that counts Robert Downey Jr. among its producers. The actress, who has earned widespread recognition for her role as Dr. McKay in the critically lauded medical drama “The Pitt,” will take on one of the film’s central parental roles opposite David Harbour.

The project already boasts an impressive ensemble. Rebecca Hall and Esmé Creed-Miles, who previously starred in the action series “Hanna,” were earlier confirmed for the film, and they will now be joined by rising talent Bodhi Rae Breathnach, known for her work on “Hamnet,” along with newcomer Hollie Hill-Pearson rounding out the family at the story’s core.

Adapted from Paul Tremblay’s novel of the same name, the film centers on a suburban family fractured when their teenage daughter’s descent into madness attracts the cameras of a reality television crew. The narrative then jumps forward fifteen years, following her younger sister Merry as she confronts the shadows of that trauma alongside a journalist determined to uncover the true nature of the malevolent force behind it all. Creed-Miles will portray Merry as an adult, while Hill-Pearson will bring the younger version of the character to life. Breathnach steps into the role of Marjorie, the older teenage sister whose breakdown set everything in motion, while Dourif and Harbour take on the roles of the parents caught in the middle of it all.

Behind the camera, the film is in capable hands. Austrian directing duo Veronika Franz and Severin Fiala, whose collaborative work on “Goodnight Mommy” and “The Devil’s Bath” has cemented their reputation as formidable voices in contemporary European horror, will both write and direct the adaptation.

On the production side, Lionsgate recently acquired the project from Fifth Season, signaling strong studio confidence in the material. The film is being produced by Daniel Dubiecki and Lara Alameddine of The Allegiance Theater, alongside Susan Downey and Robert Downey Jr. under their Team Downey banner, and David Gambino with Fifth Season. Scott O’Brien and Brandon Hill will oversee the project on behalf of Lionsgate.

For Dourif, this marks a natural escalation in a career that has been quietly building for years. Long before she became a breakout presence on “The Pitt,” which has already been renewed for a third season, she appeared in films directed by some of cinema’s most respected auteurs. Her credits include Paul Thomas Anderson’s “The Master,” Gus Van Sant’s “When We Rise,” and Christopher Nolan’s “Tenet,” a body of work that speaks to consistent recognition from filmmakers with sharp instincts for talent.

Breathnach, meanwhile, has her own exciting slate ahead. In addition to “A Head Full of Ghosts,” she is attached to a new adaptation of “Sense and Sensibility” and to Robert Eggers’ highly anticipated horror thriller “Werwulf,” making her one of the more compelling actors to watch in the genre space over the coming years.
